Your personal information will never be used for marketing purposes, but will be passed to the PSA to initiate your membership if required. We will use your mobile number to text you when your policy is due for renewal.
Please enter your anticipated turnover for the next 12 months. You must endeavour to enter an accurate figure to the best your knowledge (given previous years turnover or contracts that you already know about for the forthcoming 12 months) as this fact is considered material by insurers. It is accepted that this figure may change during the year, you only need to advise us once your policy is live if your anticipated turnover exceeds £125,000 during the policy period.

Our standard public liability policy covers unlimited work in the UK & EU, 100 days in Rest of World (no more than 90 consecutive days) with US & Canada limited to 30 days forming part of this, during the policy period. If you enter more than 30 days in the US, there is an additional fee of £52 for every 30 day increments. The maximum number of days in the US that can be purchased online is 120, so if you will be working there for longer than four months, please contact us enquiries@crewcover.com
You declare that the information disclosed in this proposal, is to the best of your knowledge and belief both accurate and complete. If you give false or misleading answers, we may be entitled to refuse to pay a claim or to trat the insurance as it never existed. Failure to disclose all material information or any change in the information advised (i.e. information likely to influence the assessment of the risk) could invalidate the insurance. If you are in any doubt whether any information is material it should be disclosed. Submitting this proposal does not bind you to enter into this insurance. We and you are entitled to choose the law that will govern this contract of insurance. We propose English law and this will apply unless otherwise agreed.
No cover is in force until this proposal is accepted by the insurer and the premium is paid. The insurer reserves the right to decline any insurance proposal or to offer different premium and terms from those quoted dependent on the information you have provided.
